I agree to attend and participate in Young Adults Conference 2025 – Melbourne on Saturday 6th December 2025 – Tuesday 9th December 2025 at Allambee Camp, 390 Mirboo-Yarragon Rd, Allambee, VIC 3871.
I agree to participate in the camp’s activities. I consent to Social Development Inc photographing and video recording me during camp activities. I further consent to Social Development Inc:
· using such photographs and recordings in presentations to parents or community members, for display within Social Development Inc premises, and for the purpose of marketing future camps and similar programs; and
· providing copies of such photographs and recordings to Participants or their parent / guardian for personal use and enjoyment, including by digital delivery after the program.
I acknowledge that once such photos or recordings are provided to Participants or their parent / guardian, Social Development Inc is not responsible for how they are subsequently used, shared, or published by third parties. These photos or recordings are provided to Participants as a personal keepsake and are to be used respectfully and in a way that considers privacy of others. Please do not distribute or publish photos involving others without their consent.
I further consent to transportation (by bus or by car) to another venue for the purposes of a camp activity (eg: transport by bus to a sport centre).

I confirm that the information I have supplied in the Young Adults Conference 2025 registration form is true and accurate and I understand that any incomplete or misleading information may affect my eligibility to participate in Young Adults Conference 2025. Should I require emergency medical treatment, I also understand that any incomplete or misleading medical information will affect the medical treatment administered.
I understand that I am obliged to correct and update the Participant’s personal and medical information where it has changed, and I accept full responsibility for any loss or injury resulting from the reliance upon any old or inaccurate information that I provide.

Social Development Inc collects personal and health information for registration to Young Adults Conference. The purpose of Young Adults Conference is to encourage participants to try new experiences, emphasise the importance of a healthy and balanced lifestyle, enhance participants’ self-esteem, critical thinking and teamwork skills and encourage expression and creativity.
If personal and health information is not collected, the planning and implementation of Young Adults Conference will not be able to be carried out effectively. Social Development Inc must have this information on hand to, amongst other things, communicate with parents and guardians during the planning and implementation of Young Adults Conference ensure the safety and security of participants and efficient medical treatment can be provided at the camp site if necessary, and to ensure the content of the camp is suitable and catered appropriately to specific cohorts of participants.
Information provided to Social Development Inc will not be disclosed to any third party without your express consent.
Social Development Inc’s privacy policy allows you to access your information anytime to make changes as required. You may contact [email protected] to arrange this access.
Social Development Inc’s privacy policy allows you to file a complaint if you believe Social Development Inc has breached any Australian Privacy Principles. In addition, you may refer any difference or dispute to The Conciliation and Arbitration Board, having jurisdiction under the constitution of The Shia Imami Ismaili Muslims.
Social Development Inc will only disclose information to overseas persons with your express consent.
All information collected is securely stored in an electronic database. Only the members of the core project implementation team of Young Adults Conference will have access to this information.

Medical
In the event of a medical emergency, I hereby authorise emergency treatment to be administered by a qualified staff member. I also give permission to be transported by ambulance to an emergency centre or hospital if needed. I further consent to medical treatments and procedures to be performed by a licensed physician or hospital when deemed immediately necessary or advisable by the physician to safeguard my health. I further accept responsibility for all expenses incurred for the medical treatment in case of an emergency.

Release
I release, indemnify and hold harmless Social Development Inc, including His Highness Prince Aga Khan Shia Imami Ismaili Council for Australia and New Zealand, its portfolios, members, staff, volunteers and agents (including Young Adults Conference 2025 faculty and staff) from all liability, claim or expense, however arising, in anyway connected my involvement with the Camp.
